fix(cli): require explicit gist cache id

Summary

• require EFFA_GIST_CACHE_ID for effa gist
• remove cache Gist discovery and automatic cache Gist creation
• initialize a missing company pointer file once without recursive retries
• add focused regression tests and update CLI documentation

Deployment note

Consumers must configure EFFA_GIST_CACHE_ID with the existing internal cache Gist ID before upgrading.

Validation

• pnpm --filter @effect-app/cli test:run
• pnpm lint-fix
• pnpm check
• IDE diagnostics: clean
